Kansas City, KS- The Care Collaborative, launched in 2014, has expanded to provide specialized care for stroke, heart attacks, sepsis, COVID-19, heart failure, and diabetes across Kansas.

Over two-thirds of healthcare organizations in Kansas are part of the Collaborative.

The initiative has significantly improved patient outcomes in rural areas, particularly in managing time-critical conditions like heart attacks and strokes.

  • Vern joined the Care Collaborative in 2020, which inspired him to keep meticulous daily records of his blood pressure, glucose levels, and weight.
  • His health data is connected via Bluetooth to nurses at the University of Kansas Health System, who call him every morning with updates.
